Rationale:
A home-based record is a record of an individual’s health status and their history of health services received (primarily maternal, neonatal and child health (MNCH)), including health, growth and development status, visits to a health care provider, and vaccinations received. Ownership of a home-based record is a good indicator of health system performance.

Method of measurement
Household surveys: Survey items correspond to children’s history in coverage surveys. The principle types of surveys are the Expanded Programme on Immunization (EPI) 30-cluster survey, the UNICEF Multiple Indicator Cluster Survey (MICS), and the Demographic and Health Survey (DHS). The indicator is estimated as the percentage of children 0-59 months of age who had a home-based record seen during a household survey.

Method of estimation:
The data are informed and constrained through household surveys conducted from 2000 onwards. Each country's data are reviewed individually; data and information are not "borrowed" from other countries. If national data are available from a single source, the data is based solely on that source. If no data are available for the most recent estimation period, the data remains the same as the previous year's. If new data or information subsequently become available, the relevant portion of the time series is updated. Although household survey provide data with documented and/or recall history, we have only used data based primarily on children aged 0-59 months who had a card seen during the household survey.
