Rational use of medicines improves health care delivery

Inappropriate and irrational use of medicines is a widespread problem that waste resources and seriously undermines the quality of patient care and can also result in antimicrobial resistance. A Drug Therapeutics Committee (DTC) is a tool for promoting more efficient and rational use of medicines by providing a forum for joint work jointly to improve health care delivery, focusing on medicines-related components.

A well-functioning DTC has been shown to be one of the most effective structures in hospitals able to address drug use problems. In the Pacific, DTC is a forum to review and update treatment guidelines and essential drug list; and develop medicine-related policies.

Strengthening the rational use of medicines

In many Pacific countries DTCs exist but they do not function effectively. Each country has its own challenges that hinder the functioning of DTC such as lack of funds, unclear TOR, infrequent meetings and lack of commitment from DTC members.

To support countries to develop effective DTCs, WHO had developed a Drug and Therapeutics Committees – A Practical Guide in 2003 in collaboration with Management Sciences for Health; this is still used by developing countries as a reference in strengthening their DTC.

The guidance provided in this manual is aimed at all kinds of DTC - whether in public or private hospitals and at all levels, from district to tertiary referral level. This manual aims to provide practical guidance to doctors, pharmacists, and hospital managers and other professional who may be serving on DTC, or who are concerned with how to improve the quality and cost efficiency of care.

The Fiji experience

Fiji was one of the countries that had requested assistance from WHO to provide copies of DTC practical guide for their use. Whilst after receiving copies of DTC practical guide, the secretary of Fiji National Medicines & Therapeutics Committee stated that, “Certainly, the Manual will serve as the guide to strengthen the role of the National Medicines and Therapeutic Committee.”

Strengthening of the performance of DTC in order to promote the rational use of medicines is also one of the key objectives highlighted in the Regional Framework for Action on Access to Essential Medicines in the Western Pacific (2011-2016).
