Integrated care for older people (ICOPE): guidance for person-centred assessment and pathways in primary care

Overview
The guidance in this handbook will help community health and care workers to put the ICOPE recommendations into practice. It offers care pathways to manage priority health conditions associated with declines in intrinsic capacity – loss of mobility, malnutrition, visual impairment, hearing loss, cognitive decline, depressive symptoms..
The primary intended audience for this handbook is health and social care workers in the community and in primary care settings. The guidance should also inform health-care workers whose specialized knowledge will be called on, as needed, to assess and to plan care for people with losses in intrinsic capacity and functional ability.
This guidance seeks to support health and social care workers in community settings to detect and manage declines in intrinsic capacity, based on WHO’s Guidelines on community-level interventions to manage declines in intrinsic capacity, and to address the health and social care needs of older adults comprehensively.
