National malaria elimination report

Overview

This template is to be used by countries that are applying to WHO for certification of malaria elimination. Certification of malaria elimination is the official recognition by WHO of a country’s malaria-free status. The information requested in this template is considered to be the minimum requirements for the national malaria elimination report. It is to be used in conjunction with the manual "Preparing for certification of malaria elimination, second edition".
