Sexual Health Assessment of Practices and Experiences (SHAPE) questionnaire

The SHAPE questionnaire consists of a set of priority questions related to sexual practices, behaviours and health-related outcomes that are relevant and comprehensible to the general population. The questionnaire is a combination of interviewer-administered and self-administered modules, intended to be used across diverse global contexts.

In collaboration with the HRP Alliance for Research Capacity Strengthening, this questionnaire was developed and refined through a global, multi-year consultative process and a multi-country cognitive interviewing study, outlined below.

Our goal: To improve the ability of researchers to collect and compare relevant data on sexual practices and sexual health-related outcomes across countries.

REDCap and XLSForm versions

This questionnaire is intended to be implemented using a combination of computer-assisted personal interviewing (CAPI) and computer-assisted self-interviewing (CASI). To facilitate this, REDCap and XLSForm versions of the questionnaire are available for free download.
